05 — Waiver & Release

Liability Waiver, Release & Assumption of Risk

1. Assumption of Risk

I understand that youth soccer training and camp activities ("the Camp") involve inherent risks of physical injury, including but not limited to sprains, fractures, collisions, heat-related illness, and, in rare cases, catastrophic injury. I voluntarily allow my child to participate in the Camp, organized and led by PowerXerg Soccer Training, Coach Thomas Mazza, and Coach Xergio Aguilar ("the Coaches"), with full knowledge of these risks.

2. Release of Liability & Hold Harmless

In consideration of my child's participation in the Camp, I, on behalf of myself, my child, our heirs, and assigns, release, waive, discharge, and covenant not to sue PowerXerg Soccer Training, Coach Thomas Mazza, Coach Xergio Aguilar, and any facility or venue hosting the Camp, including the venue hosting the Camp (Dietz Sports Field), from any and all liability, claims, demands, or causes of action arising out of ordinary negligence in connection with my child's participation, except to the extent caused by gross negligence or willful misconduct.

3. Medical Treatment Authorization

I authorize the Coaches to secure any emergency medical treatment, including transportation to a medical facility, deemed necessary for my child's immediate health and safety during the Camp. I understand the Coaches will attempt to contact me or the emergency contact listed above as soon as reasonably possible.

4. Personal Property

PowerXerg Soccer Training and the Coaches are not responsible for lost, stolen, or damaged personal property brought to the Camp.

5. Photo & Video / Media Consent

Coaches may take photos and video of Camp activities for training, promotional, and social media purposes (including but not limited to Instagram, Facebook, and the PowerXerg website). Please select one:

YES — I give permission for my child to be photographed/recorded and featured in PowerXerg social media and promotional materials.

NO — I do not give permission for my child to be photographed/recorded for social media or promotional use. The Coaches will make reasonable efforts to exclude my child from any published photos or video, including group shots.

6. Cancellation & Refund Policy

Cancellations made on or before Saturday, August 1, 2026 (2 days before Camp begins) are eligible for a 50% refund. Cancellations made after August 1, 2026, or no-shows, are not eligible for any refund. Refund requests must be submitted in writing to PowerXerg Soccer Training.

7. Late Pickup Policy

Camp ends at 11:00 AM each day, with a 30-minute pickup grace period. If your child is not picked up by 11:30 AM, a late pickup fee of $1 per minute begins at 11:31 AM and continues until pickup, charged to the parent/guardian on file. Please ensure the phone number provided at registration is the best way to reach you during camp hours so we can stay in the loop if plans change.

8. Weather Policy

PowerXerg monitors weather conditions closely and will make every effort to run camp as scheduled. If a full day (or more) of camp is cancelled due to rain or unsafe weather conditions, the Coaches will reschedule the missed training to a make-up day that same week — currently planned for Thursday, August 6th or Friday, August 7th, 2026, at the Coaches' discretion. Make-up day scheduling is not optional and is not eligible for refund; PowerXerg is committed to delivering the full 3 days of training your athlete registered for, and the specific make-up date will be communicated to the phone number and email provided at registration.

9. Code of Conduct

I understand my child is expected to follow Camp rules and Coach instructions. PowerXerg reserves the right to dismiss any athlete from the Camp, without refund, for unsafe or disruptive behavior.

10. Acknowledgment

I confirm that I am the parent or legal guardian of the athlete named above, that I have read this entire waiver, and that I am signing it voluntarily.
